At CheckMarket, we want to do something to help in these difficult times. So we took what we know and applied it to develop a COVID-19 screening form for employers with built-in notifications, reports & dashboards that companies can use to screen their own employees and visitors with their own questionnaire reporting. Our template offers a good starting point to get up and running quickly. Add your own situation specific questions, edit or remove any of the default questions and you can launch quickly.
Run your own screenings
It all runs in the browser; there is no software to install. Works on PCs, tablets, smartphones. For hygienic reasons, the best way to have people complete the screening is on their own smartphone.
You can start from our best practice questions and then modify them, remove some and add your own questions.
Either have them fill it out from home before they leave for work or place a large QR-code at the entrance that employees and visitors scan to start the screening. A shortcut can be saved to smartphone homescreens.
You can set up automated notifications that trigger when someone fails a screening. These alerts can be sent by email, text message, Slack, http request, etc… to different people depending on location or other conditions.
The reporting breaks down: passes & fails by day, location, department, symptom & other questions. You can create real-time sharable reports and dashboards for all screenings or filtered to show only screenings from a certain location.
Results screen
After answering the questions and taking their temperature, the respondents are shown either a red or green results screen. The green clearance screen contains a QR-code which can be validated by scanning it with the photo app of any smartphone. The verification will show if the clearance is still valid and not expired.

We already have this practice in our clinic since March 1 before our shift.

Since March 18th, our staff have completed pre-shift screening over 28K times using the COVID-19 screening tool. The vast majority (~85%) of staff are using their mobile phones to complete the screening, thus reducing contact with high touch surfaces like a kiosk keyboard.
